Permethrin + Crotamiton

Indications

Scabies and pruritus can both be treated with this lotion.

Pharmacology

The pyrethroid pediculocide and scabicide permethrin is a pyrethroid pediculocide and scabicide. It paralyzes and kills the pest by delaying repolarisation by limiting sodium ion input through nerve cell membrane channels.

Crotamiton exhibits antipruritic and scabicidal properties against Sarcoptes scabei. It's also an effective pediculocide.

Dosage & Administration

Adult: Permethrin & Crotamiton lotion is suitable for adults, children of 2 months of age and above, and the elderly. It is for external use only and should not be applied to broken skin, mucous membranes or near the eyes. Permethrin & Crotamiton lotion should be applied to skin which is clean, dry and cool. It should not be used immediately after a hot bath. Permethrin & Crotamiton lotion is intended for a single application over the whole body. Permethrin & Crotamiton lotion should be left on for at least 8 hours. Reapply the lotion to the hands if they are washed within 8 hours of treatment. The whole body should be washed thoroughly 8-12 hours after application. Adults & children over 2 years: Apply the lotion over the whole body but not the head and face. Pay particular attention to the areas between fingers and toes, under nails, wrists, armpits, external genitalia, breasts and buttocks.

The elderly: Apply the lotion over the whole body including the neck, face, ears and scalp. Pay particular attention to the areas between fingers and toes, under nails, wrists, armpits, external genitalia, breasts and buttocks. Avoid the area close to the eyes.

Children under 2 years: Children under 2 years should only be treated under medical supervision. Apply the lotion over the whole body including the neck, face, ears and scalp. Pay particular attention to the areas between fingers and toes, under nails, wrists, armpits, palms of hands and soles of feet, external genitals and buttocks. Avoid the area around the mouth where the lotion could be licked off and the area around the eyes.

Interaction

The use of corticosteroids to treat eczematous-like symptoms should be avoided before using permethrin because it increases the risk of aggravating the scabies infestation by lowering the immunological response to the mite. Although unlikely, interactions between the two medications could result in potentiated adverse events or decreased efficacy.

Contraindications

In patients with known hypersensitivity to permethrin or crotamiton, the combination of permethrin 5 percent and crotamiton 10 percent is contraindicated.

Side Effects

There have been complaints of a burning or stinging feeling that goes away shortly. This is usually moderate, and it happens more commonly in people who have severe scabies. Rashes, itching, erythema, edema, and eczema are all temporary signs and symptoms of skin irritation. These are widely thought to be a part of scabies' natural history.

Pregnancy & Lactation

Permethrin and crotamiton are not suggested during pregnancy since there is no evidence of their safety, especially in the first three months. It's unclear whether the active ingredient makes it into breast milk. It's best to avoid using it on the nipples if you're nursing.
