Fusidic acid + Hydrocortisone
In the cases of the following skin disorders, bacterial infections are already present or are believed to be present:
The compound cream of Fusidic Acid BP 2% and Hydrocortisone Acetate BP 1% contains the powerful local antibacterial effects of Fusidic acid and the anti-inflammatory and antipruritic effects of hydrocortisone acetate. When used externally, Fusidic acid is effective against Staphylococcus, Streptococcus, Corynebacterium, Neisseria and certain Clostridia and Bacteroides. Fusidic acid is an antibacterial agent and can be used as an inhibitor of protein synthesis in microorganisms. It interferes with the translocation step by stabilizing the G complex of ribosomal guanosine diphosphate elongation factor. This prevents the aminoacyl tRNA from binding to the ribosome and then stops transferring extra amino acids to the growing peptide. In humans, hydrocortisone is the main natural glucocorticoid. In the dose of the drug, its main effect is to reduce the skin's response to lesions (ie anti-inflammatory drugs). It also has immunosuppressive and anti-mitotic effects.
Adults: This Cream should be applied 3 times daily and gently
massaged onto the affected areas for 2 weeks. A shorter course should be
considered if symptoms improve.
Children: It is not recommended in children under 3 years of age.
Fusidic Acid may impede the metabolism of medicines that undergo significant biotransformation in the liver if taken systemically, however there is no proof for this. There have been no reports of dangerous medication interactions with topical Hydrocortisone Acetate.
Patients who are hypersensitive to Fusidic Acid, Hydrocortisone Acetate, or other ingredients in the cream should avoid using it. It's contraindicated in bacterial infections caused by non-susceptible organisms, fungal infections, tuberculosis of the skin, syphilitic skin infections, chicken pox, vaccination eruptions, and viral diseases of the skin in general, just like other topical antibiotic/corticosteroid combination preparations.
Fusidic Acid has been known to produce minor irritation at the application site, although this has seldom resulted in therapy being stopped. Hypersensitivity responses have been reported seldom. Dryness, itching, burning, local irritation, striae, skin atrophy, atrophy of subcutaneous tissues, telangiectasia, hypertrichosis, change in pigmentation, and secondary infection are examples of adverse effects. When used on the face, it might cause acne rosacea or perioral dermatitis.
It is unknown if Fusidic Acid and/or topical Hydrocortisone Acetate are safe to use during pregnancy or breastfeeding. The possible benefits of using this cream during pregnancy or breastfeeding must be balanced against the dangers to the fetus or nursing infant.
Fusidic Acid and Hydrocortisone Acetate should not be used in or near the eye because of the possibility of conjuctival irritation by Fusidic Acid. When used under occlusive dressing, over extensive areas or on the face, scalp, axillae and scrotum, suffcient absorption may occur, giving rise to adrenal suppression and other systemic effects.
